To see if you meet the diagnostic criteria for Social Anxiety Disorder, also called Social Phobia, please complete the following self-test by clicking the "yes" or "no" boxes next to each question,

Marked and persistent fear of one or more social (or performance) situations where you are exposed to unfamiliar people or possible scrutiny by others?   Yes No
Fear that you will act in a way or show symptoms of anxiety that will be humiliating or embarrassing?   Yes No

 

PART B:  

Does exposure to the the feared social situation almost always triggers  anxiety or a panic attack? Yes No
Do you recognize that the fear is excessive or unreasonable? Yes No
Do you avoid the social or performance situation or endure it with intense anxiety or distress? Yes No

 

PART C: The anxiety or avoidance of the situation causes:

Significant interference in your normal routine? Yes No
Significant interference in your functioning at your job or school? Yes No
Significant interference in your social activities or relationships? Yes No
Marked distress about having the phobia? Yes No

 

PART D: Is your fear or avoidance:

Unrelated to another disorder, such as Panic Disorder, Agoraphobia, or Separation Anxiety Disorder? Yes No
Unrelated to a physical cause, like medication use? Yes No



SCORING:  You may have a Social Anxiety Disorder (Social Phobia) if:

Part A:  You answered "Yes" to all items, and

Part B:  You answered "Yes" to all items, and

Part C:  You answered "Yes" to at least one item, and

Part D:  You answered "Yes" to all items.
